Under 1Care Health, patients and families can access a continuum of home-based services: comfort-focused hospice and palliative care through 1Care Hospice and skilled home health services including Private Duty Nursing (PDN), skilled nursing, and therapy.

Our goal is simple: the right care, in the right place, at the right time — whether a patient is managing a life-limiting illness, recovering from a hospitalization, or living with complex, ongoing medical needs.

1Care Hospice Services

Comfort-focused care for patients with life-limiting illness.

1Care Hospice provides individualized care for patients with a life-limiting illness and a prognosis of approximately six months or less who choose comfort care and symptom management rather than curative treatment.

What hospice involves

Levels of hospice care

Routine Home Care Ongoing hospice visits wherever the patient lives.
Continuous Care Short-term, intensive support at home for uncontrolled symptoms.
General Inpatient Care Inpatient management when symptoms cannot be safely controlled at home.
Respite Care Short-term inpatient care to provide rest for family caregivers.

Hospice does not provide 24-hour “live-in” care, but nurses are on-call day and night, and visits can be increased when needs change.

Eligibility for hospice

Patients are generally eligible for hospice when:

  • A physician certifies a life-limiting illness with a prognosis of six months or less;
  • The focus of care shifts from cure to comfort and quality of life;
  • The patient or legal decision-maker elects hospice services.

Who pays for hospice services?

Hospice services are covered 100% by:

  • Medicare and Medicare HMO (Part A)
  • Nevada Medicaid and Medicaid HMO

Many contracted private insurance plans also cover hospice. Some commercial policies may require a co-pay or deductible. Our team will verify coverage and explain benefits before admission.

1Care Home Health Services

Skilled care at home for recovery, chronic conditions, and medically fragile patients.

1Care Home Health provides skilled clinical care in the home for patients who are recovering from illness or surgery, managing chronic conditions, or living with complex medical needs.

Core Home Health services

  • Skilled Nursing — disease management, wound care, IV therapies, medication management, and patient/family teaching.
  • Private Duty Nursing (PDN) — extended-hour, one-on-one nursing care in the home for medically fragile patients (including pediatrics).
  • Therapy Services — Physical Therapy, Occupational Therapy, and Speech Therapy to support mobility, function, communication, and independence.
  • Home Health Aides — assistance with bathing, grooming, and activities of daily living as ordered by the physician.
  • Medical Social Work — resource coordination, psychosocial support, and long-term planning.

  • Patient’s private residence
  • Family homes and shared residences
  • Certain group homes or community settings (depending on coverage and plan of care)

Specialty programs

  • Pediatric PDN and pediatric skilled nursing at home
  • Complex chronic disease management (e.g., heart failure, COPD, diabetes, neurologic conditions)
  • DOL / Workers’ Compensation home health services (where eligible)
  • Post-hospital and post-surgical recovery support

Can my pain and symptoms be controlled at home?
Yes. Pain and many other symptoms can often be managed safely at home. Our nurses work with your physician to adjust medications and interventions, and a nurse is available 24/7 for urgent concerns.
Can I go back to the hospital and still receive hospice care?
Many issues that might lead to an emergency room visit can be managed by the hospice team at home. If hospitalization is necessary, we coordinate with the hospital and support your transition back home as soon as it is appropriate.
Does hospice provide 24-hour in-home care?
Hospice does not typically provide 24-hour “live-in” care. Instead, we schedule regular visits and remain on-call 24/7 for urgent issues. We also teach family and caregivers how to provide daily care safely and confidently.
